<p class="par4">1.	What papers do I need to get filled out?

<p>You need the claim form, the HPPA form and if you have had the policy for less than one year, you will need a list of all the doctors seen and medicines you have taken in the last year.


<p class="par4">2.	How can I push my claim through faster?

<p>The insurance company works on claims as they come in.  In other words, everything received is logged in and they take them first come first serve.  If you have all the necessary papers faxed to them the first time, it will cut a lot of mailing time out.  That is why it is so important to have these filled out COMPLETELY and the list of doctors and medicines you have taken in the past year.

<p class="par4">3.	Where do I send my claim form?

<p>You can mail it to the address on the how to file a claim page or you can fax it to them.  It is much quicker if you fax the information.  The fax no. is 1-402-479-8937 or 1-402-479-8938.  Please make sure you put Attn:  Claims Dept. on the fax or with your mail.


<p class="par4">4.	How long will it take to get my check?

<p>The average time is about 4 or 5 weeks.  If you have not had your policy for more than one year, the pre-existing will be checked out thoroughly.  This takes a little more time than if you have had the policy for awhile.  The most important thing you can do to speed up the process is to have all of the above documents filled out COMPLETELY and fax them directly to the insurance company.


<p class="par4">5.	What is the difference between an accident claim and a health claim?

<p>If you have an accident claim, there is a zero day waiting period for your disability to take effect.  Please be sure you have the doctor state the cause and effect.  For example:  Mr. Smith has lower back problems that were caused by falling off the ladder on Sept. 1st.

<p>If the doctor just puts down "lower back problems" it then becomes a health claim which has a 7 day waiting period.

<p>If you have a health claim, it has a 7 day waiting period before it begins to pay benefits.  Please be sure you get your paperwork in within 30 days of both health and accident claims.


<p class="par4">6.	Is there anyone I can call and speak to if I have any questions about my claim or how to do it?

<p>There are several different people you may call.  If you have a question on how to do things or just want someone to help you, please call Debbie Litton at 770-320-8008.  For all claims questions and follow up on a claim, the best person is Ann Spears at 1-800-662-5099.  If you want to talk directly to the claims department, please call 1-800-289-1122.  As soon as you are issued a claim number, it can easily be followed up on by contacting Ann Spears or the claims department at Professional Insurance Co.


<p class="par4">7.	Once I start getting my check, do I have to fill out paperwork every month?

<p>When you get your first check, the company will send you another form to get completed by the doctor.  It is very important to get any of these forms they may send you filled out in a timely fashion.  If they are not received it will only hold your next check up until they get what they need to continue to release your checks to you.

<p>Normally, it depends on how long your doctor has had you off to begin with.  For instance:  if the doctor says you are off indefinitely, you will have to get one filled out as they request.  If the doctor has you off for three weeks, you will have to get one in if it changes to six weeks.  Etc.   Please be sure to get them done so there is no time between checks.

MAKE COPIES OF EVERYTHING AND IF YOU FAX THE INFORMATION, KEEP THE ORIGINALS.
